Requirements
- Active Arizona PT or DPT license
- CPR certification
- AHCCCS ID (we can help you get this)
- Article 9 certification
- Dependable transportation
- Strong interpersonal skills

Responsibilities
- Conduct evaluations
- Develop treatment plans
- Deliver skilled physical therapy services
- Monitor patient progress
- Maintain timely documentation
