About The Position
The Physical Therapist works in the outpatient department at Ascension Alexian Outpatient Center in Schaumburg, Illinois, developing, implementing, and regularly reassessing individualized, patient-centered plans of care in collaboration with patients and families. The role delivers evidence-based interventions for complex diagnoses, including neurological and orthopedic conditions, and collaborates with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning. The therapist uses innovative rehabilitation technologies and best practices to maximize functional recovery and independence. The schedule is rotating and PRN, with a minimum weekend or evening and holiday requirement.
Requirements
- Bachelor's degree required.
- Physical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation before hire or job transfer; a valid license pending letter is accepted until the license is issued.
- BLS Provider certification obtained within 30 days of hire or job transfer; American Heart Association or American Red Cross certification is accepted.
Nice To Haves
- Master's or doctorate degree preferred.
- One year of outpatient care experience.
