About The Position

This is a licensed professional position in Physical Therapy responsible for treating injured, ill, or disabled individuals through therapeutic use of everyday activities to help individuals develop, recover, improve and maintain the skills needed for daily living. The position averages 40 hours/month and requires the use of an Electronic Healthcare Record (EHR) system. The contractor personnel shall adhere to Department Operation Regulations (DORs), client policies and procedures, and ICF guidelines. The role involves independent direction of screening, assessment, treatment, intervention, administration, program planning, and documentation for acute and long-term care patients to optimize their functional abilities. It requires applying clinical reasoning in developing physical therapy plans and programs, understanding principles for implementing treatment plans and using adaptive equipment. The ability to interpret physician's directions, utilize evidence-based treatment options, and plan/supervise subordinate staff is necessary. The role also involves planning, organizing, and adapting therapeutic activities, communicating regularly in writing with various stakeholders, implementing and assisting in the development of treatment plans, and escorting individuals as assigned. Responding to healthcare needs by observing and reporting changes in patient condition, behavior, and emotional response is also a key responsibility.

Responsibilities

  • Utilizes Electronic Healthcare Record (EHR) system.
  • Follows Department Operation Regulations (DORs), the client policies and procedures, and ICF guidelines.
  • Acts with independence in directing screening and assessment, treatment, intervention, administration, program planning, and documentation for acute and long-term care patients to enable them to improve their optimal functional abilities.
  • Applies clinical reasoning in developing physical therapy plans and programs.
  • Knowledge of principles and methods used to develop and implement individual treatment plans and the use of adaptive equipment designed to assist in self-help tasks.
  • Ability to understand, interpret, and adapt a physician's written or oral directions utilizing evidence-based treatment options; and to plan, assign, and supervise the work of subordinate therapists and aides.
  • Ability to plan, organize, lead, and adapt planned therapeutic activities in accordance with patient's needs.
  • Communicates timely and regularly in writing with individuals, the Interdisciplinary Team (IDT), co-workers, and/or supervisors utilizing agency forms, computer programs and email as required.
  • Implements and carries out individual treatment/therapy plans and assists in developing and teaching living skills related to self-help, recreation, socialization, meals, etc. and escorts individuals as assigned.
  • Responds to health care needs for individuals by observing, reporting changes in physical condition, behavior and emotional response.
