About The Position

PARS Therapy is seeking a compassionate, skilled Physical Therapist to join its home health team in Nederland, Texas. The role involves providing patient-centered, personalized, evidence-based care in patients’ homes to help them regain movement, reduce pain, and improve quality of life. The position requires a Texas Physical Therapist license and professional liability insurance specific to that license. A graduate degree or higher in Physical Therapy from an accredited program is required. Home health or rehabilitation experience is preferred.

Requirements

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage specific to the Texas Physical Therapy license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Ability to adapt care approaches to suit a variety of patient personalities and needs.
• Up-to-date knowledge of evidence-based treatment techniques and best practices.
• Excellent communication skills and a friendly, professional demeanor when educating and interacting with patients.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
• Provide therapeutic interventions, including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with physicians, nurses, occupational therapists, and other interdisciplinary team members.
• Conduct home safety assessments and recommend measures to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
