About The Position

We are seeking a Physical Therapist to provide skilled home health services in Seaside, OR, and surrounding areas. This is a full-time, day shift position with 8-hour shifts. The role involves providing 1:1 patient care, with the expectation of visiting up to 6 patients per day and meeting a standard of 30 productivity points. The therapist will be responsible for charting using EPIC Remote Client for Home Health and Rover (phone app) for Oasis documentation. Mileage reimbursement will be provided at the IRS standard rate.

Requirements

• 1 year of Skilled Home Health Experience.
• BCLS certification.
• Valid driver's license and reliable transportation.
• Proficiency in EPIC charting.
• Experience with Oasis documentation.
• Primary series COVID vaccination plus one booster.
• Ability to work business casual attire, no jeans.
• Must have their own transportation.

• Locals accepted within a 50-75 mile radius.

Responsibilities

• Provide skilled physical therapy services to patients in their homes.
• Manage a caseload of up to 6 patients per day.

• Minimize drive and windshield time by effectively grouping clients.
• Adhere to charting and attire requirements.
