About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in Catonsville, Maryland, and surrounding communities. Care is delivered in private residences and senior living settings, including independent living, assisted living, and memory care, using FOX’s Geriatric House Calls™ model. Clinicians help patients improve mobility, independence, wellness, and quality of life through evaluations, individualized plans of care, and evidence-based treatment. Telehealth and other virtual care may also be incorporated when appropriate. The role offers autonomy to create and manage a treatment schedule, build lasting patient relationships, and collaborate with an interdisciplinary team. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. The stated standard Maryland base pay range for full-time roles is $70,000–$110,000 annually, with the opportunity to earn more; part-time and FOX Flex compensation is prorated. Benefits are available for eligible roles. H1B sponsorship is available for qualified clinicians, and new graduates may be eligible for FOX’s Emerging Professionals Mentor Program.
Requirements
- Valid Physical Therapy license in the state or states of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, assisting with patient transfers and bed mobility, repositioning patients, standing and walking for extended periods, climbing stairs, and performing therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Ability to wear personal protective equipment when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living communities, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with an interdisciplinary team of physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
- Use opportunities to support clinical team growth and care delivery, such as referring colleagues or patients, teaching, mentoring, supervising assistants, and providing care above quota, as applicable.
