About The Position

The physical therapist is responsible for patient evaluations, care planning, directing, and administering physical therapy modalities of treatment as prescribed by a licensed physician. The PT administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability. The PT assists patients in reaching their maximum performance and level of functioning, while learning to live within the limits of their capabilities. The physical therapist ensures patient charges are accurate and submitted in a timely manner. Physical Therapists coordinate, delegate and supervise responsibilities assigned to support staff (PTA, OT /PT Aide, etc.). The physical therapist participates in operational aspects of the department, contributing to performance improvement and CQI activities. Physical therapists participate in all infection control, departmental equipment training, organizational safety and fire safety programs.

Responsibilities

  • Performs appropriate patient assessments resulting in an effective treatment plan.
  • Reassesses patients at appropriate intervals of time demonstrating the ability to observe treatment effect and modify the plan of care as indicated.
  • Involves the patient and/or family in the assessment process and the formation of the care plan.
  • Provides care in accordance with physician’s orders.
  • Demonstrates appropriate case management to ensure quality of care and best patient outcome.
  • Communicates appropriately to physicians, nurses, case managers, and other members of the health care team to ensure care is collaborative and appropriate.
  • Assesses and documents patient outcomes criteria at the initial and discharge evaluations. (all shoulder, back, incontinence, and anodyne).
  • Demonstrates knowledge of physical therapy modalities / treatments and performs correctly.
  • Demonstrates knowledge of and competency in using equipment pertinent to providing physical therapy services.
  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Includes the family in teaching as appropriate.
  • Documentation meets current standards and policies.
  • Documentation is legible.
  • No inappropriate abbreviations are used.
  • Documentation is completed in a timely manner.
  • Medical Records are closed as per policy.
