About The Position

Wellstar is seeking a Physical Therapist to join their Inpatient Rehab team. This role involves assessing, planning, organizing, and participating in rehabilitative programs to improve patient mobility, relieve pain, increase strength, and prevent deformity in patients suffering from disease or injury. Services are provided across the full continuum of care in a cost-conscious, quality-focused, and customer-oriented manner, ensuring compliance with all applicable regulatory standards.

Requirements

  • Graduate from an accredited physical therapy program
  • Bachelor's Degree in Physical Therapy
  • Master's Degree in Physical Therapy
  • Doctorate in Physical Therapy
  • Physical Therapist license
  • Basic Life Support (BLS) or BLS - Instructor or BLS - Provisional
  • Effective communication skills
  • Customer service focused
  • Team oriented
  • Basic computer skills

Responsibilities

  • Assess, plan, organize, and participate in rehabilitative programs.
  • Improve mobility, relieve pain, increase strength, and decrease or prevent deformity.
  • Collaborate with physicians for the establishment and ongoing evaluation of treatment programs, plan of care, goals, and discharge plans.
  • Establish a plan of care in conjunction with the patient and/or family members.
  • Establish realistic discharge plans incorporating patient and/or family member goals.
  • Implement the physical therapy treatment plan.
  • Select appropriate treatment activities to progress patients towards goals.
  • Revise goals/plan of care with patient/family input.
  • Provide ongoing patient/family education and training.
  • Provide discharge instructions, follow-up, and referral to community resources as appropriate.
  • Act as a referral source for staff, families, and physicians on services and equipment related to rehabilitation services.
  • Document evaluation and plan of care according to departmental guidelines.
  • Document each treatment session according to departmental protocols.
  • Complete and submit family education, discharge, and other required documentation within facility and program guidelines.
  • Adhere to ethical business practices.

  • Maintain confidentiality of all patient information.
  • Maintain attendance/punctuality.
  • Adhere to departmental dress code policy.
