We are a unique option for individuals 55 and older who qualify for nursing facility level of care and wish to remain in their homes as they age. What does that mean? We care for the elderly at our day center on Garfield near Hammond during regular business hours and as needed in their home. We began serving the elderly in 2019 and our service model is so popular it’s growing quickly!

Requirements

  • Minimum of 1 year experience working with the frail or elderly.
  • Masters or a Doctor of Physical Therapy degree.
  • CPR certified.
  • Valid Mi driver's license.

Responsibilities

  • Provide coordination of care and provide training to family members, and PACE North staff.
  • Going to our participants homes and providing home care as well as providing care at our center, With lots of one on one care for our participants.
  • Help in the discharge planning for our participants from the hospital, assisted living, or skilled nursing facilities.
