About The Position
The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions. This role involves developing and revising plans of care based on resident needs, establishing functional goals, outcomes, and discharge plans. The therapist will also plan for resident discharge, anticipate long-term and/or short-term resident needs, and educate other healthcare team members, residents, family, and caregivers on information regarding the care plan for the residents.
Requirements
- Current state licensure in Physical Therapy, in good standing
- Bachelor’s degree
Responsibilities
- Develops and revises plan of care based on the residents’ needs
- Establishes functional goals, outcomes, and discharge plans
- Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs
- Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents
