About The Position

Our mission is to help seniors live independently, confidently, and joyfully through compassionate in-home care, personalized therapy, and innovative brain health programs. We are seeking a Physical Therapist to provide in-home care for Medicare Part B patients. This role involves helping patients regain mobility, manage pain, and live independently, all from the comfort of their homes.

Requirements

Active Physical Therapy (PT) license in the state of practice.
Experience in home health or outpatient therapy preferred.
Knowledge of Medicare Part B documentation and regulations.
Strong assessment, treatment planning, and communication skills.
Reliable transportation and valid driver's license.
Comfortable working independently and managing a flexible schedule.
This position requires Level 2 background screening through Florida's Care Provider Background Screening Clearinghouse.

Nice To Haves

Experience in home health or outpatient therapy preferred.

Responsibilities

Assess patients in their homes and create personalized treatment plans.
Provide skilled therapy to improve mobility, strength, balance, and independence.
Educate patients and caregivers on exercises, safety, and fall prevention.
Document progress and ensure compliance with Medicare Part B regulations.
Collaborate with the home health/ therapy team to coordinate care.
Adjust treatments based on patient progress and needs.
