Human Touch Home Health is seeking a New Graduate Physical Therapist to join their team serving Blacksburg, Christiansburg, Radford, Pulaski, Giles County, and surrounding New River Valley communities. This role is designed for recent graduates looking for a supportive environment that invests in their future, fosters professional development, and provides mentorship to build confidence as a clinician. Home health offers a unique opportunity to develop clinical skills, build meaningful patient relationships, and see the direct impact of care as patients regain mobility, independence, and confidence. Unlike busy outpatient settings, this role allows for focused, individualized patient care while working independently with the support of an experienced clinical team.
Requirements
- Graduate of an accredited Physical Therapy program
- Active Virginia Physical Therapist license
- Valid driver's license and reliable transportation
- Strong communication and organizational skills
- Passion for helping patients achieve their goals
Responsibilities
- Complete evaluations, reassessments, and discharge visits
- Develop individualized treatment plans
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on exercises and safety techniques
- Collaborate with physicians and interdisciplinary team members
- Complete documentation accurately and on time
