About The Position
Human Touch Home Health is seeking a licensed Physical Therapist (PT) to join our growing team serving patients throughout Quakertown, Coopersburg, Upper Bucks County, and surrounding Pennsylvania communities. This is a PRN opportunity that offers flexibility, autonomy, and the ability to focus on meaningful patient outcomes. As a Home Health Physical Therapist, you'll provide care in patients' homes, helping them recover from surgery, illness, injury, hospitalization, and mobility challenges by improving strength, balance, safety, and independence. The role emphasizes meaningful patient care and making a lasting impact, without the constraints of crowded clinics or double-booked schedules.
Requirements
- Active Pennsylvania Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical, Communication, and Documentation Skills
- Commitment to Delivering Exceptional Patient-Centered Care
Nice To Haves
- Home Health Experience Preferred but Not Required
Responsibilities
- Complete evaluations, reassessments, and discharge visits
- Develop individualized treatment plans based on patient goals and needs
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on mobility, exercise programs, fall prevention, and safety strategies
- Collaborate with physicians and interdisciplinary team members
- Maintain timely and accurate clinical documentation
