About The Position
Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Carbon County, Pennsylvania, with a primary focus on Jim Thorpe, Lehighton, and surrounding communities. This PRN opportunity offers flexibility, autonomy, and the ability to make a meaningful difference in the lives of patients. As a Home Health Physical Therapist, you will help patients recover from illness, injury, surgery, and hospitalization by improving strength, mobility, balance, and confidence in their daily lives. Your expertise will help patients remain safe in their homes, avoid unnecessary hospitalizations, and return to the activities they enjoy most.
Requirements
- Active Pennsylvania Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical, Communication, and Documentation Skills

- Home Health Experience Preferred but Not Required

Responsibilities
- Complete evaluations, reassessments, and discharge visits
- Develop individualized treatment plans based on patient goals and needs
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on exercise programs, mobility training, fall prevention, and safety strategies
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with physicians and interdisciplinary team members
- Maintain timely and accurate clinical documentation
