About The Position
Unlimited Health Care Services Inc. is seeking a compassionate and skilled Physical Therapist (PT) to join their home health team in Fort Lauderdale and East Broward County. This is a per diem, 1099 opportunity offering flexibility for PTs who want greater control over their schedules, additional income, or to work around existing commitments. The role involves working directly with patients in their homes to improve mobility, strength, balance, function, and independence, fostering meaningful relationships and making a difference in patients' quality of life. New graduates are encouraged to apply.
Requirements
- Active Florida Physical Therapist (PT) license in good standing
- Doctor of Physical Therapy (DPT) from an accredited program
- Current CPR/BLS certification
- Strong clinical assessment and problem-solving skills
- Excellent communication and interpersonal skills
- Valid driver's license and reliable transportation for patient home visits
- Current auto insurance
- Ability to work independently and manage your visits responsibly
- Compassionate, patient-centered approach to care

Responsibilities
- Perform comprehensive physical therapy evaluations and assessments
- Develop individualized treatment plans based on each patient's needs and goals
- Provide skilled physical therapy interventions in the home setting
- Implement therapeutic exercises and activities to improve strength, mobility, balance, coordination, and functional independence
- Evaluate patient progress and modify treatment plans as appropriate
- Educate patients and caregivers on therapeutic exercises, mobility, positioning, safety, and home exercise programs
- Instruct patients and caregivers in techniques to promote safe mobility and independence
- Communicate patient progress and changes in condition with physicians, nurses, and other members of the care team
- Complete accurate and timely clinical documentation for each visit
- Participate as an active member of the interdisciplinary care team
