Functional Pathways is currently hiring at our newest location, Wesley Willows in Rockford, IL, for a full-time Physical Therapist. Wesley Willows is a senior living community in Rockford, IL, that has been serving seniors since 1966. Requirements
- Bachelor's, Master’s or Doctorate degree in physical therapy from an accredited program.
- Certification and licensure requirements as established by state and national board associations.
- Active state license required.
- Excellent organizational and time management skills
- Excellent written and verbal communication skills
- Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines.
- Ability to supervise Physical Therapy Assistants
- Completion of all required screenings, certifications, and licensure.
Responsibilities
- Screen, evaluate, re-evaluate, discharge plan, and make revisions in any established plan of care as supervised by the physician.
