About The Position

Responsible for the daily management of assigned patient caseload, ensuring timely care and documentation to meet patient needs and services as prescribed in the established plan of care and per physician orders. This role involves collecting data from patient assessments, analyzing it to determine issues and resources, and identifying patients at high risk for re-hospitalization or falls. The Physical Therapist will evaluate patients to determine rehabilitation needs, develop a plan of care in conjunction with the physician, and implement therapy services. They will also direct care coordination with a multidisciplinary team, including supervising LPTAs and HHAs, and maintain communication with the team and physicians regarding patient conditions and orders. Therapy will be incorporated into short and long-term goals, with documentation in visit notes, progress notes, and discharge summaries. The role requires adherence to state practice acts, Medicare guidelines, and agency policies.

Requirements

  • Doctorate degree in Physical Therapy from an accredited school or education requirements commensurate with the date of graduation.
  • Maintains continuing education requirements as mandated by applicable state professional standards.
  • Physical Therapy License in the applicable state or South Carolina.
  • Basic Life Support for Healthcare Provider certification from the American Heart Association required.
  • Current state driver's license and auto insurance at all times.
  • Personal vehicles must be well maintained and in safe driving conditions.
  • Demonstrates knowledge of principles of growth and development.
  • Demonstrates skills and competency appropriate to the ages, culture, developmental stages, and special needs of the patient.
  • Work requires walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
  • Must be able to lift and support the weight of 50 pounds in handling patients, medical equipment, and supplies.
  • Ability to travel extensively during all seasons to assigned patient's place of residence.
  • Works in a variety of home environments and conditions.
  • Must speak English in good understandable terms.
  • Intact sense of sight, hearing, smell and touch.
  • Finger dexterity.
  • Critical thinking and the ability to concentrate.
  • Must be able to respond quickly to changes in patient and/or unit conditions.
  • Ability to work flexible schedule to meet patient's needs.
  • Personal Protective Equipment such as gloves, goggles, gowns, and masks are sometimes required due to possible exposure to hazardous chemicals or blood and body fluids.

Nice To Haves

  • 1-year experience as a licensed PT in a clinical care setting or home health preferred.
  • 1 year pediatric experience within the past 5 years preferred or education and training may be provided by Advocate Health At Home.

Responsibilities

  • Daily management of assigned patient caseload, ensuring timely care and documentation.
  • Collect data pertinent to the healthcare consumer's health from patient assessments, telephonic contacts, and EMR information.
  • Analyze data to determine issues and available resources for effective resolution.
  • Identify patients at high risk for re-hospitalization, falls, or high utilization of resources requiring additional intervention.
  • Prioritize multi-issue situations based on criticality of patient health, welfare, safety, and impact to services.
  • Evaluate PT patients to determine rehabilitation needs and potential for achievement.
  • Develop a plan of care in conjunction with the patient's physician.
  • Establish and implement the plan of care, perform PT services certified by the physician.
  • Direct the care coordination of a multidisciplinary clinical team approach.
  • Supervise LPTAs and/or HHAs involved in patient care.
  • Maintain communication with the multidisciplinary team, providing updates on schedules, patient data, needs, and physician orders.
  • Incorporate short and long-term goals into therapy, as evidenced by documentation.
  • Support effective communication and collaboration with physician oversight and branch supervision.
  • Perform ordered treatments, therapies, and/or procedures in accordance with State Physical Therapy Practice Act, Home Health conditions of Participation, JCAHO, Medicare Guidelines, OASIS, and agency policy and procedures.
