Responsible for the daily management of assigned patient caseload, ensuring timely care and documentation to meet patient needs as prescribed in the established plan of care and per physician orders. This role involves collecting patient data, analyzing it to determine issues and resources, and identifying patients at high risk for re-hospitalization or falls. The Physical Therapist will evaluate patients, develop and implement a plan of care, and direct the care coordination of a multidisciplinary clinical team, including supervising LPTAs and HHAs. Communication with physicians and the branch supervision is crucial for timely feedback on patient conditions and plan of care changes. The therapist must adhere to all state and federal regulations, including the State Physical Therapy Practice Act, Home Health conditions of Participation, JCAHO, Medicare Guidelines, OASIS, and agency policies and procedures.
