About The Position

Sequoia Care is a leading provider of rehabilitative and long-term care services in Florida, serving residents in senior living, skilled nursing, and home health settings. Our Physical, Occupational, and Speech Therapy teams are dedicated to delivering compassionate, evidence-based care focused on improving residents' function and quality of life. We are looking for licensed Physical Therapists (PTs) to join our expanding team. In this role, you will be responsible for assessing, developing, and implementing personalized treatment plans to help residents regain movement, reduce pain, and restore independence. Requirements

  • Doctorate or Master’s degree in Physical Therapy from an accredited program
  • Current Physical Therapist license in the state of Florida (required)
  • Ability to be accredited by all insurance payers
  • CPR certification (as required by facility or state)
  • Strong clinical, documentation, and communication skills
  • A passion for resident-centered care and improving quality of life

Nice To Haves

  • Senior living or similar experience preferred

Responsibilities

  • Perform evaluations and develop individualized treatment plans
  • Deliver skilled physical therapy services to residents in senior living setting
  • Monitor and document resident progress, adjusting plans as needed
  • Educate residents, caregivers, and staff on mobility, exercise, and prevention strategies
  • Collaborate with interdisciplinary teams to ensure coordinated, high-quality care
  • Ensure compliance with all applicable Florida licensure and documentation standards
