About The Position

Hartford HealthCare's Family Health Center Rehab is a multidisciplinary clinic seeking a Physical Therapist (PT) passionate about outpatient neuro settings. This role will contribute to growing the Neuro program in the Willimantic area. The PT will administer medically prescribed Physical Therapy treatments, independently evaluate patient function, determine treatment strategies, and deliver or delegate services to address physical dysfunction or pain. This includes evaluation skills, patient analysis, and therapeutic application of agents, exercise prescription, and other procedures to maximize functional independence.

Requirements

  • Graduate of Physical Therapy Program from an accredited American Physical Therapy Association school, BS, master's or doctoral degree.
  • CT active or temporary license.

Nice To Haves

  • Neuro experience preferred, however, will accept new grads with strong neuro clinical experience.
  • Ability to work independently and drive the neuro program.

Responsibilities

  • Administer medically prescribed Physical Therapy treatments to patients.
  • Independently evaluate patient’s level of function.
  • Determine treatment strategies.
  • Deliver and/or delegate Physical Therapy services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain.
  • Perform evaluation skills and techniques.
  • Analyze patients.
  • Apply therapeutic physical and chemical agents.
  • Prescribe exercise and other procedures to maximize the patient's functional independence.
  • Evaluate patients with neurologic diagnoses, including Parkinson’s Disease, CVA, SCI, concussion, MS, balance and fall prevention, and create individualized Plan of Care.
  • Demonstrate competence in amplitude based exercises (LSVT BIG or PWR!) to address movement disorders.
  • Provide patient and/or caregiver training in Safe Steps balance and fall prevention strategies.
  • Provide appropriate and safe exercise plan for patients with neuro diagnoses, with modifications to functional ability as needed.
  • Work in a multidisciplinary team including PT, OT, SLP.
