About The Position
Choice Health at Home is seeking a compassionate and skilled Physical Therapist to provide individualized care that helps patients regain mobility, improve function, manage pain, and maintain independence in the comfort of their own homes.
Requirements
- Graduate of an accredited Physical Therapy program.
- Current Physical Therapist license in the state of practice.
- CPR certification required.
- Valid driver's license and auto insurance.
Nice To Haves
- Home Health experience preferred.
- HomeCare HomeBase experience preferred.
Responsibilities
- Perform comprehensive patient evaluations and develop individualized treatment plans.
- Implement therapeutic interventions to restore function, improve mobility, and reduce pain.
- Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
- Collaborate with physicians, nurses, caregivers, and interdisciplinary team members.
- Educate patients and families on treatment goals and home exercise programs.
- Maintain timely, accurate documentation and comply with agency policies and regulatory requirements.
- Follow infection control and patient safety standards.
