About The Position

At Suncrest At Brighton/Suncrest Hospice, our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating the resources to increase our staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. The Physical Therapist will be responsible for providing care and services for a group of patients, visiting patients where they reside, which could include private homes, Independent Living Facilities, and Assisted Living Facilities. This position is a contributing member of the interdisciplinary team and evaluates and services to the home health patient with neurological, musculoskeletal, cardiopulmonary impairments; gross motor, sensorimotor, and developmental delays; as well as traumatic injuries and orthopedic disorders.

Requirements

  • Current Physical Therapist licensure (in good standing) in the state(s) of practice.
  • One year of experience under license.
  • Ability to work well within a team environment.
  • Ability to work independently and on your own from a day-to-day basis.
  • Understanding of state and federal rules and regulations.
  • Time management skills.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, valid driver’s license, and auto insurance.
  • CPR Certification.

Responsibilities

  • Providing care and services for a group of patients.
  • Visiting patients where they reside, which could include private homes, Independent Living Facilities, and Assisted Living Facilities.
  • Evaluating and servicing home health patients with neurological, musculoskeletal, cardiopulmonary impairments; gross motor, sensorimotor, and developmental delays; as well as traumatic injuries and orthopedic disorders.
  • Collaborating daily with a team of clinicians.
  • Working closely with patients on a 1-1 level.
  • Managing visits, documentation, and meetings.
  • Responding to urgent needs by prioritizing and triaging multiple concerns.
  • Driving to patient locations.
