About The Position

BAYADA Home Health Care is seeking compassionate and motivated new graduate Physical Therapists for openings throughout Delaware (New Castle County, Kent County, Sussex County). This role offers a structured orientation, dedicated clinical support, and experienced PT mentors to help new graduates transition from academia to independent practice. Home health provides an opportunity to connect with patients one-on-one, develop meaningful relationships, and help individuals remain safely and independently in their homes. BAYADA ensures new graduates are supported throughout their first year with experienced clinicians and clinical managers to build confidence, strengthen skills, and foster professional growth.

Requirements

• Graduate of an accredited Doctor of Physical Therapy (DPT) program.
• Licensed or license-eligible Physical Therapist in the state of practice.
• New graduates are strongly encouraged to apply.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and assess patients' functional status and mobility.
• Develop individualized treatment plans to maximize independence and safety.
• Provide skilled therapeutic interventions in patients' homes.
• Educate patients, families, and caregivers on exercises, mobility, fall prevention, and home safety.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Complete timely electronic documentation while maintaining the highest standards of patient care.
