About The Position

The Hospital At Home Department is seeking a Full-Time Physical Therapist to provide essential home-based healthcare services within the Jacksonville community. This role involves patient care in their homes, with mileage reimbursement for travel within the Jacksonville metro area. Additionally, the Physical Therapist will offer support to Baptist Health's acute care rehab units as needed, coordinating with leadership. The ideal candidate will possess experience in both acute care and home health settings, with a preference for those interested in program development. This position is based at Baptist Medical Center and requires a strong background in orthopedic and neurological physical care.
Requirements
- 1 or more year(s) of home health Physical Therapy Experience strongly preferred
- 1 or more year(s) of acute care Physical Therapy Experience strongly preferred
- Orthopedic Experience
- Bachelor's degree (or higher) in Physical Therapy Required
- Licensed Physical Therapist from FL Department of Health Required
- Basic Life Support (BLS) certification from the American Heart Association Required
- At least 1 year of recent experience working in an acute care setting
- Home health patient care experience
- Experience providing direct orthopedic and neurological physical care

- Candidates with experience and interest in program / new service development are preferred.
Responsibilities
- Evaluates, plans, directs and administers physical therapy treatment to patients.
- Provides patient care to help improve or restore function and prevent disability following injury, disease or physical disability.
- Oversees support staff.
- Participates in departmental operations and in performance improvement activities.
- Responsible for remaining within budget guidelines.
- Participates in mandatory education and licensure requirements.
