We are hiring a Physical Therapist for our Hospital At Home Department in Jacksonville, Florida. This PRN Days position involves providing patient home-based healthcare throughout the Jacksonville community. The schedule requires at least 3 shifts per month, Monday - Friday, with no weekend or holiday commitments. This role includes commuting to patient homes within the Jacksonville metro area with mileage reimbursement. The Physical Therapist will also provide floating coverage support to Baptist Health acute care rehab units as scheduling permits. The ideal candidate will have experience in both acute care and home health settings, with a preference for candidates experienced in program/new service development. This position is part of the Rehab Services Team at Baptist Medical Center and requires at least 1 year of recent experience in an acute care setting, home health patient care experience, and direct orthopedic and neurological physical care experience. Requirements

1 or more year(s) of home health Physical Therapy Experience strongly preferred
1 or more year(s) of acute care Physical Therapy Experience strongly preferred
Orthopedic Experience
Bachelor's degree (or higher) in Physical Therapy Required
Licensed Physical Therapist from FL Department of Health Required
Basic Life Support (BLS) certification from the American Heart Association Required
At least 1 year of recent experience working in an acute care setting
Home health patient care experience
Experience providing direct orthopedic and neurological physical care

Candidates with experience and interest in program / new service development are preferred.

Responsibilities

Evaluates, plans, directs and administers physical therapy treatment to patients.
Provides patient care to help improve or restore function and prevent disability following injury, disease or physical disability.
Oversees support staff.
Participates in departmental operations and in performance improvement activities.
Responsible for remaining within budget guidelines.
Participates in mandatory education and licensure requirements.
