Home Care Physical Therapist- Full Time (Eastern Suffolk)
About The Position
Metro Physical & Aquatic Therapy is seeking a motivated and passionate full-time Physical Therapist to provide in-home care. This is an opportunity for a Physical Therapist ready to advance their career within a growing organization that offers a progressive, supportive, and innovative team environment. All experience levels are welcome, and training will be provided for the right new graduate. The role emphasizes patient care with a quick EMR system (no Oasis required), offering great work-life balance, flexibility, and autonomy to see patients any day of the week. Preference will be given to candidates who can see patients in their primary geographic area.
Requirements
- Physical Therapist Program Graduate
- Licensed Physical Therapist in the state of New York or eligible.
- Ability to travel to patients’ homes within the assigned geographic territory
Nice To Haves
- Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
Responsibilities
- Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
