The successful candidate will utilize clinical expertise to manage patient care through examination, evaluation, planning, implementation, and re-assessment.

Requirements

  • Bachelor's Degree Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program).
  • Current and valid NYS Physical Therapy license and registration.
  • 1- 2 years Hospital (Inpatient) or Orthopedic (Outpatient) or Pediatric setting.
  • Excellent communication (both oral and written), critical thinking, and customer service skills.
  • Basic computer skills.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Nice To Haves

  • Master's Degree Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program).
  • Post Doctoral/Doctorate in Physical Therapy (DPT) Physical Therapy (NYS Registered or APTA Accredited Physical Therapy Program).
  • 1 - 2 years Home Care experience.

Responsibilities

  • Performs an objective functional evaluation and re-assessment of patient including the use of appropriate tests and measures; patient safety issues and home environment factors; and establishes a plan of care with measurable and functional short term/long term goals.
  • Performs other duties as assigned.
  • Assists Agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participates in Agency quality assessment/performance improvement initiatives.
  • Completes and maintains accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education, intervention and evaluation according to Federal, Department of Health (DOH) and The Joint Commission (TJC) requirements.
  • Re-assesses patient progress toward short term/long term goals and modifies the plan of care accordingly.
  • Educates patient and family regarding patient s functional status and plan of care. Provides a home exercise program and instruct on home safety factors.
  • Interacts and collaborates with other members of the healthcare team to optimize patient outcomes and health care goals.
  • Implements and modifies appropriate interventions/treatments as indicated based on the patient's functional status, diagnosis, age, patient response and status in conjunction with the plan of care.
