The physical therapist contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. This role involves assessing and evaluating patients' therapeutic, rehabilitative, and functional status, developing total plans of care, and directing physical therapy treatments. The therapist will also instruct patients and families on the use of therapeutic appliances, determine priority needs, report patient reactions to treatment, and initiate physical therapy programs. Additionally, the role includes training patients in the use of prosthetic devices, identifying needs for other home health services, preparing clinical summaries, participating in discharge planning, and providing consultations to families/caregivers. Communication responsibilities include providing in-service education and participating in peer consultation. The position also involves supervising Physical Therapy Assistants and instructing/supervising home health aides.
