About The Position

FOX Rehabilitation is expanding in Marshalltown, IA, and surrounding areas, seeking licensed Physical Therapists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. The position involves working in four partner Senior Living Communities: Glenwood Place and the Willows in Marshalltown, Oakview Nursing home in Conrad, and Westbrook Acres in Gladbrook. It's ideal for Physical Therapists who value autonomy, flexibility, and meaningful patient outcomes, with the freedom to manage their own schedules and build strong patient relationships. Clinicians may also utilize telehealth and virtual care for greater flexibility and continuity of care. This is a high-impact PT role aimed at positively impacting the lives of the communities served.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain
Degree from an accredited Physical Therapy program
Basic computer and EMR proficiency
Current CPR certification
Exerting medium physical effort, including lifting 25–50 lbs occasionally, 10–25 lbs frequently, and up to 10 lbs constantly
Performing CPR
Providing moderate to maximum assistance with patient transfers and bed mobility
Repositioning patients, standing/walking for extended periods, climbing stairs, and performing therapeutic activities
Driving to patient homes and senior living communities as part of the Geriatric House Calls™ model
Wearing PPE when required

Responsibilities

Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
Document effectively within FOX’s EMR system in accordance with regulatory standards
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
Provide telehealth / virtual care when appropriate to extend access and flexibility
