About The Position

FOX Rehabilitation is expanding in Brooklyn, NY, and seeking dedicated, licensed Physical Therapists to provide exceptional one-on-one care to older adults in their homes or within Senior Living Communities. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model. It's an ideal opportunity for Physical Therapists who value autonomy, flexibility, and impactful patient outcomes, with the freedom to manage their own schedules and build strong patient relationships. The role may also incorporate telehealth and virtual care, enhancing flexibility and patient support. This is a high-impact PT position aimed at positively influencing the lives of the communities served.

Requirements

Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
Degree from an accredited Physical Therapy program.
Basic computer and EMR proficiency.
Current CPR certification.

Responsibilities

Provide one-on-one physical therapy to older adults in private residences, Senior Living Communities, Independent Living, Assisted Living, and Memory Care settings.
Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
Deliver therapy sessions focused on patient wellness, longevity, mobility, and functional independence.
Document effectively within FOX’s EMR system in accordance with regulatory standards.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
Provide telehealth / virtual care when appropriate to extend access and flexibility.
