About The Position
We are looking for a motivated Physical Therapist to join our integrated medical practice in Fort Wayne, IN. We are looking for a Physical Therapist who is passionate about working closely with patients, creating long term relationships with them while providing high-quality, individualized care. Ideally, the incoming Physical Therapist is a confident team player who enjoys collaborating with an integrated team for high quality, comprehensive patient care. We are an integrative physical medical clinic serving patients in the Fort Wayne area and we are well respected and known for helping our patients improve the quality of their health. Requirements
- Graduation from an accredited Physical Therapy Program
- Active Physical Therapy license in IN
Responsibilities
- Patient Consultation-review health and medical histories, exam patients, review pertinent medical records/imaging, and reason for visit
- Consult with referring Physicians/Healthcare team regarding diagnosis as needed
- Develop and implement physical therapy treatment programs based on each patient's particular needs
- Assist patients with all phases of physical therapy treatment and techniques to include exercise instruction, modern modalities, and activities of daily living
- Patient education as to diagnosis, treatment plan options, pre and post treatment care, lifestyle modifications, take-home stretches and exercises, and benefits of continued care
- Documentation and Charting (timely and accurate notes)
- Work cooperatively with the multidisciplinary team and other support staff for comprehensive patient care
- Patient follow up to ensure treatment goals are being met- reassessments
