About The Position
FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in private residences and senior living communities in Mahtomedi, Minnesota, and surrounding communities. Using FOX’s Geriatric House Calls™ model, clinicians help patients improve mobility, independence, wellness, and quality of life. Care may also include telehealth and other virtual services when appropriate. The role offers clinician autonomy, the ability to create and manage a treatment schedule, and opportunities to build lasting patient relationships. FOX offers full-time, part-time, and PRN/FOX Flex employment pathways. Full-time positions are salaried with benefits; part-time benefits eligibility depends on a minimum-hours requirement; PRN/FOX Flex compensation is per unit. A sign-on bonus is available to those who qualify. H-1B sponsorship is available for qualified clinicians, and new graduates may participate in FOX’s Emerging Professionals Mentor Program. The standard Minnesota base pay range for a full-time role is $60,000–$105,000 annually, with the opportunity to earn more. Part-time and FOX Flex compensation is prorated. Requirements
- Hold a valid Physical Therapy license in the state(s) of practice, or be eligible to obtain one.
- Have a degree from an accredited Physical Therapy program.
- Have basic computer and EMR proficiency.
- Hold current CPR certification.
- Be able to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers, repositioning, extended standing and walking, stair climbing, and therapeutic activities.
- Be able to wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with physical therapists, occupational therapists, speech-language pathologists, and fitness specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
