About The Position

As a fulltime therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. Assess and document patients' physical strengths to support physician evaluation and guide treatment goals. Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team. Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. Document patient observations, interventions, and evaluations promptly and thoroughly. Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Requirements

  • Degree from an accredited Physical Therapy Program (approved by the APTA)
  • Current and unrestricted Physical Therapy license
  • Current CPR certification
  • Experience collaborating with a team of healthcare professionals
  • Valid state driver's license
  • Maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
