About The Position
FOX Rehabilitation is hiring a licensed Physical Therapist to provide one-on-one care to older adults in a Senior Living Community in Falmouth, Massachusetts. The role uses FOX’s Geriatric House Calls™ model, delivering evidence-based care to help patients improve mobility, independence, wellness, and quality of life. Clinicians may also provide care in private residences and use telehealth or other virtual-care options when appropriate. The position offers autonomy to create and manage a treatment schedule, develop lasting patient relationships, and collaborate with an interdisciplinary team. FOX offers full-time, part-time, and PRN/Fox Flex employment pathways. A sign-on bonus is available to those who qualify. New graduates may be eligible for FOX’s Emerging Professionals Mentor Program, and H1B sponsorship is available for qualified clinicians. FOX is a clinician-operated practice focused on meaningful relationships, evidence-based care, clinician-driven decision-making, and its mission to abolish ageism. In Massachusetts, the stated standard base pay range for a full-time role is $70,000–$100,000 annually, with an opportunity to earn more. Part-time and PRN compensation is prorated. Requirements
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain one.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
- Ability to meet the stated physical demands of in-home and community-based care, including lifting, CPR, patient assistance and transfers, repositioning, extended standing and walking, stair climbing, and therapeutic activities.
- Ability to drive to patient homes and senior living communities.
- Wear PPE when required.

Responsibilities
- Provide one-on-one physical therapy to older adults in private residences and senior living settings, including independent living, assisted living, and memory care.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Provide therapy that supports patient wellness, longevity, mobility, and functional independence.
- Document care in FOX’s EMR system in accordance with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
- Provide telehealth or virtual care when appropriate.
- Drive to patient homes and senior living communities as part of the Geriatric House Calls™ model.
