About The Position
Human Touch Home Health is seeking a licensed New Graduate Physical Therapist to join our growing team serving patients throughout Blacksburg, Christiansburg, Radford, Pulaski, Floyd, Giles County, Montgomery County, and surrounding New River Valley communities. This role offers mentorship, support, flexibility, and the opportunity to build lasting relationships with patients. The position focuses on providing meaningful time with patients, developing personalized treatment plans, and seeing the life-changing impact of care. Experienced clinical leaders are committed to helping new graduates transition from the classroom to confident practice.
Requirements
- Active Virginia Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Communication and Interpersonal Skills
- Passion for Delivering Compassionate, Patient-Centered Care

Responsibilities
- Complete physical therapy evaluations and reassessments
- Develop individualized treatment plans based on patient goals
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
- Collaborate with physicians and the interdisciplinary care team
- Monitor patient progress and adjust treatment plans as needed
- Complete timely and accurate clinical documentation
