About The Position
Human Touch Home Health is seeking a licensed New Graduate Physical Therapist to join our growing team serving patients throughout Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, and surrounding communities. This full-time opportunity offers mentorship, clinical support, and the freedom to grow while making a real difference in patients' lives. As a Home Health Physical Therapist, you will work one-on-one with patients in their homes, developing personalized treatment plans to help them regain strength, improve mobility, and return to their desired quality of life. You will have the time to build relationships, celebrate patient milestones, and learn alongside experienced clinical leaders. The role emphasizes meaningful patient care, supportive mentorship, and a career built on purpose, without the pressures of crowded clinics or double-booked appointments.
Requirements
- Active Virginia Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Communication and Interpersonal Skills
- Passion for Delivering Compassionate, Patient-Centered Care

Responsibilities
- Complete physical therapy evaluations and reassessments
- Develop individualized treatment plans based on patient goals and clinical needs
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety
- Monitor patient progress and adjust treatment plans as needed
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
