About The Position

This position is part of a new at home program that will serve outpatient therapy across the Northern Charlotte area. The Physical Therapist evaluates patients, selects and administers assessment techniques, and synthesizes information to establish an appropriate patient care plan. This includes necessary modalities, procedures, exercise programs, therapeutic groups, and patient education as warranted by a holistic view of the patient's diagnosis, condition, age, and discharge needs. The role involves gathering and interpreting appropriate data from the medical record, physician reports, patient, and/or family. Establishing functional goals in conjunction with the patient and providing evidence-based intervention to work towards meeting these goals with ongoing assessment to adjust the plan of care as needed is crucial. Documentation of evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges is required. Communication of patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff, coordination of patient's schedule and treatment programs, and discharge planning are key responsibilities. The Physical Therapist also provides supervision of PTA's and Rehab Tech/Aide as required for licensure.

Requirements

  • Bachelor's degree in Physical Therapy program required.
  • Applicable state license for Physical Therapy required.
  • BLS required per policy guidelines.
