About The Position

Care IV Home Health, a locally owned and operated home health agency in Hot Springs, Arkansas, is seeking a part-time or PRN Physical Therapist. The role provides care to patients in their homes and assisted living facilities, with responsibilities including developing and following therapy care plans, performing physical therapy, documenting progress, and completing patient admissions, discharges, and OASIS assessments. The therapist updates the Therapy Director on patient status and performs other assigned PT duties.

Requirements

• Be actively licensed or registered as a Physical Therapist in Arkansas.
• Have previous acute physical therapy experience.
• Provide a minimum of three professional references on the application or upon initial contact.

Responsibilities

• Visit patients in their homes and assisted living facilities.
• Develop and follow physical therapy care plans.
• Perform physical therapy.
• Document and chart patient progress.
• Complete patient admissions and discharges.
• Perform OASIS assessments.
• Update the Therapy Director on patient status.
• Perform other physical therapy duties as assigned.
