About The Position
AllCare.ai is expanding its care model to include physical therapy services for residents of assisted living facilities (ALFs) and senior communities across the Los Angeles area. We are seeking a licensed Physical Therapist to join our team. This role will focus on restoring mobility, improving strength and balance, and reducing fall risk for our senior patient population.
Requirements
- Doctor of Physical Therapy (DPT) degree.
- Active California PT license in good standing.
- Bilingual required: Korean and English.
Nice To Haves
- Experience with senior/geriatric populations.
- Comfortable working independently across multiple facility locations.
Responsibilities
- Deliver in-home and on-site physical therapy services to residents across assigned ALF/senior living facilities.
- Conduct evaluations, develop individualized treatment plans, and manage fixed-episode rehabilitation programs (typically 6-week care episodes).
- Document all visits accurately and in compliance with billing/coding requirements.
- Coordinate with primary care providers (NPs, physicians) on shared patients.
- Travel between facilities within the LA service area on a structured schedule.
