The Physical Therapist will evaluate patients, select and administer assessment techniques, and synthesize information to establish an appropriate patient care plan. This includes necessary modalities, procedures, exercise programs, therapeutic groups, and patient education based on a holistic view of the patient's diagnosis, condition, age, and discharge needs. The role involves gathering and interpreting data from medical records, physician reports, and patients/families, establishing functional goals with the patient, and providing evidence-based interventions. Documentation of evaluations, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, and discharge notes is required. The Physical Therapist will also communicate patient progress to physicians, referral sources, and other clinical staff, coordinate patient schedules and treatment programs, and assist with discharge planning. Supervision of PTA's and Rehab Tech/Aide as required for licensure is also a key function.
