About The Position

The Physical Therapist will evaluate patients, select and administer assessment techniques, and synthesize information to establish an appropriate patient care plan. This includes necessary modalities, procedures, exercise programs, therapeutic groups, and patient education, considering the patient's diagnosis, condition, age, and discharge needs. The role involves gathering and interpreting data from medical records, physician reports, and patient/family input. Establishing functional goals with the patient and providing evidence-based interventions are key. Documentation of evaluations, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges is required. Communication with physicians, referral sources, and other clinical staff regarding patient progress, coordination of schedules and treatment programs, and discharge planning is essential. The Physical Therapist will also supervise PTA's and Rehab Tech/Aides as required for licensure.

Requirements

  • Bachelor's degree in Physical Therapy program required.
  • Applicable state license for Physical Therapy required.
  • BLS required per policy guidelines.

  • Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff.
  • Coordinates patient's schedule and treatment programs, and discharge planning.
  • Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
  • Demonstrates any appropriate exercise and activities to patients/caregivers.
