About The Position
Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Carbon County, Pennsylvania. This PRN opportunity offers flexibility, professional independence, and the chance to make a meaningful impact by helping patients regain their independence, strength, mobility, balance, and confidence in the comfort of their own homes. The role emphasizes one-on-one patient care, individualized treatment plans, and collaboration with a healthcare team, distinct from the fast-paced environment of outpatient clinics.
Requirements
- Active Pennsylvania Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment, Communication, and Documentation Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
Responsibilities
- Complete physical therapy evaluations, reassessments, and discharge visits
- Develop individualized plans of care based on patient goals and clinical needs
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians, nurses, and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
