About The Position

Provides physical therapy and education to clients, referring them to appropriate resources as needed. May case manage clients inside and outside the assigned geographic region. The position is in Allina Health’s Home Care department and provides ongoing oversight of client plans of care.

Requirements

• Bachelor’s degree in Physical Therapy from an APTA-accredited program.
• 2 to 5 years of experience in a rehabilitation or long-term care setting.
• Minnesota Physical Therapist license required upon hire when providing services in Minnesota.
• Must have a current BLS certification from the American Heart Association, a current BLS certification from the American Red Cross, or complete Allina in-house BLS training within 30 days of hire.
• Valid driver’s license required upon hire. Minnesota and Wisconsin residents must obtain a valid driver’s license in their state of residence within 60 days of employment.

Nice To Haves

• Master’s degree in Physical Therapy.
• 0 to 2 years of experience in a home care or acute care setting.

Responsibilities

• Provide physical therapy treatment to clients.
• Initiate client assessments based on diagnosis.
• Consult with clients about their goals and desired outcomes.
• Educate clients, family members, and others involved in care about treatment plans, including age-specific and cultural considerations.
• Identify goals and interventions for each client problem, impairment, or functional limitation.
• Continuously assess client needs and functional deficits and progress the plan of care.
• Initiate appropriate referrals to other disciplines.
• Case manage clients as needed.
• Establish and provide ongoing oversight of plans of care.
• Make interdisciplinary referrals.
• Facilitate communication among involved disciplines, providers, and payer sources.
• Manage caseload priorities, scheduling, and client urgency across assigned and other geographic areas.
• Perform other duties as assigned.
