About The Position

This position integrates subjective and objective data to establish a treatment plan, utilizing exercise, physical agents, and manual techniques to address deficits. The Physical Therapist will maintain a thorough record of services provided with appropriate coding and billing, and communicate with other disciplines and referring providers to enhance outcomes. This role also involves performing thorough and timely electronic documentation, participating actively in the PI and CQI process, and assisting in the orientation, training, and mentoring of new partners. The Physical Therapist will also participate in cost-saving initiatives within the department and take an active role in the performance improvement process. A high standard of patient care and effective communication between physical therapists, assistants, and the physician group is expected. Additionally, the role includes planning and conducting lectures and training programs on physical therapy and related topics for medical staff, students, and community groups.

Requirements

  • Graduate of an accredited school of Physical Therapy
  • Licensed as a physical therapist in WI, IL or both if required
  • CPR/BLS required within 30 days of hire
  • Passing the Driver’s License Check and/or Credit Check (for those positions requiring).
  • Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check.
  • Must be able to follow written/oral instructions.
