About The Position

Requirements
- Graduate of an accredited Physical Therapy program
- Current Physical Therapy license
- CPR certification
- Valid driver’s license and auto insurance
Nice To Haves
- Minimum 1 year home health experience preferred
- HomeCare HomeBase experience preferred
Responsibilities
- Perform and document comprehensive evaluations to identify patient needs and develop individualized treatment plans
- Plan and provide therapeutic interventions to restore function, improve mobility, relieve pain, and prevent disability
- Monitor patient progress and modify treatment plans to maximize outcomes
- Maintain timely, accurate documentation per company guidelines
- Communicate effectively with patients, families, physicians, and interdisciplinary team members
- Follow infection control and universal precaution standards
- Support agency philosophy and commitment to quality patient care
