About The Position

Independence Rehab is seeking a compassionate PRN Physical Therapist (PT) who finds purpose in helping older adults regain mobility, confidence, and dignity. As a contract therapy provider partnering with Independence Rehab to treat patients at Park Forest, you'll support residents navigating recovery, chronic conditions, and life transitions - providing therapy that truly changes daily life. What a Day Looks Like As a PRN PT in our geriatric programs, your day is relationship-driven, collaborative, and clinically meaningful. You'll treat a medically complex geriatric population in a setting where clinical reasoning, compassion, and flexibility are valued.

Requirements

  • Active Colorado State Physical Therapist license in good standing
  • Graduate of an accredited PT program
  • Interest in geriatric care and functional outcomes

Nice To Haves

  • SNF experience preferred but not required

Responsibilities

  • Performing evaluations and contributing to individualized plans of care as needed
  • Guiding residents through mobility milestones - first steps, safe transfers, returning home
  • Leading gait, balance, strengthening, and functional training sessions
  • Helping reduce fall risk and build confidence with daily activities
  • Supervising and collaborating with PTAs when scheduled
  • Adjusting care plans based on patient progress and clinical judgment
  • Working alongside nursing, OT, and speech to support whole-person recovery
  • Providing reliable coverage for vacations, caseload fluctuations, and program growth
