As a Home Care Physical Therapist covering primarily for Summit and Portage Counties at Cleveland Clinic, you will utilize various treatments and exercises to help patients increase strength, range of motion, endurance and coordination in the comfort of their homes. Since this position requires close interaction, you will have the opportunity to build strong patient relationships and enhance their overall experience, in the comfort of their own home. Often, your patients are dealing with pain, so sensitivity is important. By showing compassion and patience, you can assist patients in regaining the drive they need to work hard and heal. A caregiver in this position works 40 hours per week from 8:00am to 4:30pm.

Requirements

  • Bachelor’s Degree or certificate
  • Graduate of a Physical Therapy Program accredited by the American Physical Therapy Association
  • Current state of Ohio licensure as a Physical Therapist (PT)
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
  • Valid Ohio Driver’s License (VDL) and proof of $100,000/$300,000 automobile insurance coverage

Nice To Haves

  • One year of clinical experience as a Licensed Physical Therapist in an acute, rehab or skilled nursing setting
  • Homecare or outpatient nursing experience

Responsibilities

  • Evaluate and conduct medically prescribed physical therapy treatment programs.
  • Perform initial and ongoing assessments.
  • Establish, revise and evaluate plans of care.
  • Perform physical therapy interventions utilizing standard physical therapy techniques and skills appropriate for the patient’s age and condition.
  • Supervise the activities of physical therapy assistants, students and other support caregivers.
  • Maintain complete and up-to-date documentation and files at all times regarding patient condition and the effectiveness and timeliness of care and interventions.
  • Participate in ongoing interdisciplinary assessments of assigned patients.
