About The Position

We are seeking an experienced Physical Therapist (PT) for an outpatient rehabilitation setting within a leading acute care hospital system. Candidates with experience in outpatient therapy and rehabilitation are encouraged to apply.

Requirements

• Minimum 3 years of experience required
• Active Physical Therapist license required
• Pending licenses not accepted

Nice To Haves

• Outpatient rehabilitation experience preferred
• Local candidates accepted

Responsibilities

• Evaluate and develop individualized treatment plans for patients with musculoskeletal, neurological, and functional impairments.
• Provide evidence-based therapeutic interventions to improve mobility, strength, balance, and overall function.
• Educate patients and caregivers on treatment plans, home exercise programs, and injury prevention.
• Document evaluations, treatments, progress notes, and discharge summaries accurately in the EMR.
• Collaborate with physicians and interdisciplinary healthcare teams to optimize patient outcomes.
