About The Position

We are seeking an experienced Outpatient Physical Therapist to join an innovative adult care program focused on helping older adults maintain independence and continue living safely within their communities. This role involves working closely with a geriatric population through comprehensive rehabilitation services in an outpatient setting.

Requirements

• Active Physical Therapist License (pending license considered)
• BCLS Certification required
• Minimum 1 year of experience working with seniors required
• Epic experience required
• First-time travelers not accepted

Nice To Haves

• Experience with geriatric patients highly preferred

Responsibilities

• Evaluate and treat adult and geriatric patients with a variety of physical and functional impairments.
• Develop individualized treatment plans to improve mobility, strength, balance, endurance, and overall function.
• Provide patient and caregiver education to support safety and independence.
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes.
• Document evaluations, treatment plans, progress notes, and discharge summaries in Epic.
• Support patients in maintaining the highest possible level of function within the community.
