About The Position
We're seeking a compassionate, motivated Full-Time Physical Therapist (PT) to join our growing rehabilitation team in Oregon, Ohio. This role involves evaluating and treating patients in a post-acute/skilled nursing setting, developing individualized treatment plans, collaborating with other healthcare professionals, completing documentation, and educating patients and caregivers.
Requirements
- Licensed Physical Therapist (PT) in the State of Ohio
- New graduates are encouraged to apply
- Strong communication and interpersonal skills
- Passion for patient-centered care
- Ability to work collaboratively with an interdisciplinary team

Responsibilities
- Evaluate and treat patients in a post-acute/skilled nursing setting
- Develop individualized treatment plans focused on improving mobility, strength, balance, and independence
- Collaborate with Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and families
- Complete accurate, timely documentation
- Educate patients and caregivers on treatment plans and home exercise programs
- Help patients achieve meaningful functional outcomes
